SWS Multicultural Access Project (SWS MAP)
The South West Sydney Multicultural Access Project (SWS MAP) provides support, resources and training to aged care service providers in order to enhance access to services for people from culturally and linguistically diverse backgrounds.
This Project is funded by the Department of Health and Aged Care.
The aim of the project is to ensure that CHSP target groups are aware of, and receive, culturally appropriate services to ensure that older people can live independently in their own home with appropriate care. The Project supports culturally diverse communities in building their capacity to negotiate services appropriate to their needs.
The South West Sydney Multicultural Access Project worker (MAP)
- Supports the aged care sector to connect with multicultural communities
- Supports workers, organisations and our funding agencies in planning, training or identification of needs of multicultural frail aged communities
- Supports multicultural communities to learn about and access a range of service to remain at home and engaged in their community
