SWS Multicultural Access Project (SWS HACC MAP)
The South West Sydney Multicultural Access Project (SWS MAP) provides support, resources and training to Aged and Disability service providers in order to enhance access to services for people from culturally and linguistically diverse backgrounds.
This Project is funded by the Department of Health & NSW Department of Families and Community Services, Ageing, Disability and Home Care.
The aim of the project is to ensure that CHSP, HCP and CCSP target groups are aware of, and receive, culturally appropriate services to ensure that older people and people with disabilities can live independently in their own home with appropriate care. The Project supports culturally diverse communities in building their capacity to respond to the needs of the HACC target groups.
